What pillow height actually means
Pillow height, which the bedding industry usually calls loft, is simply the distance from the surface of your mattress to the top of the pillow when your head is resting on it. That last part matters more than most people realize, because a pillow that measures six inches tall sitting empty on your bed might compress to three inches under the weight of an adult head, and it is the compressed number that your neck experiences all night long.
This is why comparing pillows by their box dimensions is close to useless. A soft polyester pillow and a dense memory foam pillow can both measure five inches on the shelf and deliver completely different support, because one collapses almost entirely and the other holds most of its shape. When you are working out your ideal pillow height, you are really asking a different question, which is how much space needs to be filled between your mattress and your head to keep your neck in a straight line with the rest of your spine.
That phrase, a straight line, is the whole game. Imagine looking at yourself from behind while you sleep on your side. Your spine should run in one continuous line from your tailbone up through your shoulder blades and out through the top of your head. No dip, no arc, no sideways bend at the neck. A pillow exists for exactly one reason, which is to fill the gap that would otherwise force that line to bend. Everything else about a pillow, the cover, the fill, the cooling technology, the price, is secondary to whether it fills that gap correctly.
Why the wrong pillow height causes pain you feel in the morning
Your neck contains a group of small stabilizing muscles that spend the day holding your head upright against gravity. Your head weighs somewhere between ten and twelve pounds, roughly the weight of a bowling ball, and those muscles are working constantly to keep it balanced on top of your spine. When you lie down at night, the whole point is that those muscles finally get to switch off, because the mattress and the pillow are now carrying the load instead.
That handover only happens if your head is properly supported. If your pillow is too tall, your neck is bent upward toward the ceiling and the muscles on one side stay lengthened while the muscles on the other side stay shortened, and neither group can fully relax. If your pillow is too flat, your head drops below the line of your spine and the same problem happens in reverse. Either way, those small stabilizing muscles keep working through the night instead of resting, and by morning they are tight, sore and slow to loosen.
Morning neck stiffness is rarely an injury. It is usually just muscles that were never allowed to clock off, holding a low grade contraction for seven straight hours because the pillow underneath them was the wrong height.
This explains something that confuses a lot of people, which is why the pain is worst on waking and eases through the morning. If your neck genuinely had a structural problem, movement would tend to make it worse over the course of the day. When the cause is postural and the pillow is at fault, the opposite happens, because once you get up and start moving, circulation returns to those muscles and the tightness gradually releases. How to measure your own pillow height in two minutes
The measurement you need is the horizontal distance from the outer edge of your shoulder to the side of your neck, because that is precisely the gap a pillow has to fill when you lie on your side. You do not need special equipment, only a soft tape measure or a ruler and a wall.
Stand with your back flat against a wall in the clothes you normally sleep in, without shoes. Have someone measure straight across from the wall to the outermost point of your shoulder, which is the bony point at the top of your upper arm. Then measure from the wall to the side of your neck at about the height of your earlobe. Subtract the second number from the first, and what remains is your shoulder-to-neck gap. For most adults this falls somewhere between four and six and a half inches, though broader framed people can exceed seven.
That figure is your starting target for pillow height when you sleep on your side. If you sleep mainly on your back, your target is considerably lower, usually between three and four and a half inches, because on your back the only gap you need to fill is the natural hollow behind your neck rather than the whole width of your shoulder. If you are a genuine stomach sleeper, your target is lower again, often under three inches, since anything taller cranks your head backward.
Write the number down and take it seriously, because it is the single most useful piece of information you can carry into a pillow purchase. It converts a vague preference into a specification, and specifications are much harder for marketing copy to talk you out of.
The variable almost nobody accounts for: your mattress
Here is where the arithmetic gets more interesting, and where most pillow buying advice on the internet quietly stops. Your shoulder-to-neck gap is a measurement taken while standing against a rigid wall. Your bed is not rigid. Depending on how soft your mattress is, your shoulder sinks into it by some amount every night, and every inch your shoulder sinks is an inch of pillow height you no longer need.
On a plush pillow top mattress, a heavier person's shoulder can sink an inch and a half or more, which means their effective gap at night is dramatically smaller than the number they measured standing up. If they buy a pillow matched to their standing measurement, they will spend every night with their head propped too high, wondering why an expensive pillow is giving them a stiff neck. On a firm mattress the opposite is true, because the shoulder stays almost entirely on the surface, the gap remains close to the standing measurement, and a pillow that felt fine on a soft bed will suddenly feel far too flat.
The practical adjustment is straightforward. If your mattress is soft or has a thick pillow top, subtract roughly one inch from your measured gap. If your mattress is medium, use your measured number as it stands. If your mattress is genuinely firm, use your measured number and lean toward the taller end of any range you are choosing between. This one adjustment resolves an enormous share of the confusion people have about why a pillow that worked beautifully at a hotel feels wrong at home, or why the pillow a friend raves about does nothing for you.
How your sleep position changes the target
Most of us are not loyal to a single position all night. Sleep studies consistently show that the average adult changes position somewhere between ten and twenty times between lights out and morning, which means the pillow you choose is really being asked to serve two or three different geometries rather than one.
Side sleeping needs the most height because the shoulder gap is the widest gap your body creates. Back sleeping needs moderate height, and crucially it needs the height to be concentrated under the neck rather than under the skull, because what you are supporting is the natural forward curve of the cervical spine rather than the back of your head. Stomach sleeping needs the least height of all, and honestly needs almost none, because your head is already turned sideways and any elevation adds rotation on top of rotation.
If you are a combination sleeper who genuinely moves between side and back, you have two reasonable options. The first is to pick a pillow sized for the side sleeping position, since that is where poor support does the most damage, and accept that back sleeping on it will feel slightly tall. How to tell your current pillow is too high
You do not always need a tape measure, because your body reports the problem fairly clearly if you know what to listen for. A pillow that is too tall pushes your head forward and up, which closes the angle at the front of your neck and opens it at the back. The symptoms follow from that geometry with some consistency.
The most common complaint is a tight, compressed ache across the back of the neck and the top of the shoulders, often worse on waking and easing within an hour of getting up. Many people also report a sore jaw or a dull tension headache starting at the base of the skull, because the muscles running from the skull down into the upper back are being held in a shortened position all night. Some people notice their throat feels dry or their breathing seems restricted, which follows from the chin being tipped toward the chest and narrowing the airway.
There is also a simple visual test. Have someone photograph you from behind while you lie on your side in your normal sleeping position, or set your phone on a timer against a stack of books. If your head is visibly tilted upward, so that the line from your spine bends toward the ceiling at the neck, your pillow height is too great for your frame or your mattress.
How to tell your current pillow is too low
A pillow that is too flat lets your head sag toward the mattress, bending your neck in the opposite direction. The symptoms overlap somewhat with the too tall pattern, which is part of why people struggle to self diagnose, but there are distinguishing signs.
People on too flat a pillow tend to report pain concentrated on one side of the neck rather than across the back of it, because the side that is stretched takes the worst of the load. Give any new pillow a real adjustment window

Four mistakes that keep people on the wrong pillow
The first mistake is buying by firmness instead of by height. Firmness describes how much a pillow resists compression, while height describes how much space it fills. The second mistake is keeping a pillow far past its useful life.
